Is a cotton gusset enough?

A small cotton panel in a synthetic pair. Is that the answer, or the least you could do?
Pick up almost any pair of underwear in a shop and you will find the same design. A synthetic body, and a small panel of cotton sewn into the crotch. The gusset.
There is a reason behind that little bit of cotton, and here's why.
What the research actually found
The most cited piece of work here is a 2019 survey study of 986 women, published in the Journal of Obstetrics and Gynaecology Research, comparing underwear habits against rates of urogenital infection.
Two findings came out of it.
The first is the one that gets less attention and is genuinely useful: the style of underwear predicted nothing. Thongs were not associated with urinary tract infections, bacterial vaginosis, or yeast infections. The received wisdom on that point turns out not to be supported.
The second is that women wearing underwear without a cotton crotch reported more yeast infections. That's an association in self-reported data, not proof the fabric caused it.
So in that data, the cut didn't track with infection. The fabric did. And the fabric it measured was the crotch.
Read plainly: a cotton gusset in a synthetic pair is a defensible reading of that study. If someone tells you the research proves you need an entirely cotton pair, they are going further than the evidence does, and you are entitled to ask them for the study.
I would rather tell you that than pretend otherwise. It is also not the end of the argument.
Where the gusset argument runs out
The case for a cotton panel and a synthetic everything else rests on a reasonable-sounding idea: the gusset covers the part that matters, so the rest is just fabric.
Fair enough on coverage. On a lot of briefs a cotton gusset does sit over the vulva. But "covered by cotton" and "the rest doesn't matter" are two different claims, and it's the second one that we feel needs closer inspection.
The study never tested it. B-THONG compared cotton crotch against non cotton crotch. It did not compare a cotton gusset against an all cotton pair. So it cannot tell you whether the panel alone is as good as the whole pair. Nobody has run that comparison. An untested question is not a settled one.
The body of the garment still sets the microclimate. Even with a cotton gusset, the rest of the garment wraps the whole area in whatever it is made of. A breathable body keeps that space cooler and drier. A synthetic one traps heat and moisture around skin that is already warm and covered. Over sixteen hours that is a comfort difference you can feel, gusset or no gusset.
Most of the fast drying evidence is about gym clothes. The case for synthetics rests on drying speed and moisture management, and it is usually illustrated with sweat, activewear and the hour after a class. That is a real scenario and synthetics have a real case in it. It is not the same scenario as a plain pair worn for sixteen hours on an ordinary Tuesday. Conclusions drawn from the first do not automatically transfer to the second.
The part that is inconvenient for me
Cotton has a genuine weakness here and I am not going to skip it.
Cotton holds a great deal more water than polyester or nylon, and it dries far more slowly. That is measurable and not in dispute. If your mental model is "moisture against skin is the problem," then a fabric that absorbs and holds moisture looks like a strange solution.
The counter, and the reason clinicians still say cotton, is that absorbency is the point. Cotton draws moisture off the skin and holds it in the fabric. Synthetics do not absorb, so the moisture stays where it was, against you, along with the heat. One OB-GYN quoted in Healthline's guide puts it as wanting underwear that will "gently absorb any extra moisture," which is a different model of the problem than "keep the fabric dry."
Both models are reasonable. Which one matters more, for everyday wear specifically, has not been settled. Anyone presenting either as obvious is telling you more about what they sell than about the evidence.
So what do you do with this
Not a verdict. A trade off, the same as the elastane one.
If you are wearing something for ninety minutes and sweating hard in it, fast drying has a real argument. If you are wearing something for sixteen hours a day, every day, against tissue that is more permeable than the rest of you, then breathability across the whole garment is worth more than a panel.
One option people forget, because the idea is a hard one to hold, is wool. Fine merino manages sweat and odour well and doesn't feel clammy the way a damp cotton pair can, which is the exact complaint synthetics were brought in to answer. Most of us picture an itchy jumper. Merino next to the skin is a different thing, and for the sport case it's one natural-fibre answer that isn't cotton.
What I would actually ask, standing in the shop:
- What is the body of the garment made of, not just the panel?
- Is this designed for sport or for every day? The right answer genuinely differs.
- Is it making a performance claim? A finish had to be applied to deliver that, and the risk tracks the claim rather than the fibre.
- If it is cotton, is it just cotton, or organic and GOTS? Worth being clear here: this whole piece is arguing about fibre choice, cotton against synthetic. That is a separate question from how the cotton was grown and processed. Conventional cotton is a chemical-heavy crop, and "cotton" on its own says nothing about that. The honest caveat
I sell underwear, and the pair I'm building is a cotton one, made with GOTS organic cotton and a 100% cotton gusset, so weigh this accordingly.
The study supports cotton where it touches. It does not prove a whole cotton pair beats a cotton panel, and I am not going to tell you it does. My opinion is that a garment worn against this tissue for most of your life is worth a little more thought than the minimum defensible standard.

It just does a quieter job, and it is mostly a comfort difference.
Are thongs bad for you?
The evidence doesn't support that. In the 2019 survey, thong use wasn't linked to urinary tract infections, bacterial vaginosis or yeast infections. A 2005 study measuring tight-fitting and string underwear found it made negligible difference to the vulvar skin's humidity, pH and microflora. On what we have, style seems to matter less than fabric.
Is cotton or synthetic underwear better?
It depends on how long you wear it and what for. Synthetics dry faster, which matters for sport. Cotton breathes and absorbs moisture off the skin, which is why it's commonly recommended for everyday wear. For most people this is a comfort and preference question more than a medical one.
Is wool underwear a good alternative?
For sport, yes, and it's underused. Fine merino manages sweat and odour well and doesn't feel clammy when it's damp, which is the main knock on cotton for exercise. The hurdle is mental: most people picture a scratchy jumper, not underwear. Merino next to the skin is soft and fine, and it's a natural fibre, so it's the non-synthetic option for the sweaty scenario.
Does cotton stay damp?
Cotton holds considerably more water than polyester or nylon and dries more slowly. That is true. The clinical view is that absorbing moisture away from the skin is desirable, which is a different objective from keeping the fabric itself dry.

A note on what I have left out: I have not quoted a figure for how much better an all cotton pair performs than a cotton gusset, because no one has measured it. The absence of that number is the honest answer to the question in the title.
